If you have made a like-kind (i.e. non-cash) contribution to the League of Women Voters of Collier County, please complete the Like-Kind Donation Form.

If you have incurred costs on behalf of the LWVCC and would like to be reimbursed for them, please complete the Request for Reimbursement Form.

If you have collected and deposited funds on behalf of the LWVCC, please complete the Record of Deposit Monies Form.

Each form, when completed, should be mailed to:
